Job Summary
Case Manager to join Better Living Health and Community Services on a one-year contract in Toronto. Responsibilities include conducting in-home psychosocial assessments, developing client-centered care plans, providing ongoing case management, navigation of healthcare system and community resources, collaboration with primary care providers and community partners, participation in outreach and committees, supervising students, participating in interdisciplinary case discussions and quality improvement initiatives, and maintaining accurate documentation in line with organizational standards. Qualifications include a BSW, registration with OCSWSSW, at least 2 years of case management experience in community health or social services, knowledge of PHIPA/privacy, and strong assessment, advocacy, and communication skills. Salary range $52,000–$57,000 annually. Location: 1 Overland Drive, Toronto, ON. In-person remote arrangement.
Required Qualifications
- Bachelor of Social Work (BSW) degree from an accredited School of Social Work
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum two (2) years of case management experience in a community health, social service, or healthcare setting
- Knowledge of PHIPA and other privacy/consent requirements
- Strong assessment, counselling, advocacy, and care coordination skills
- Excellent interpersonal, communication, organizational, and time-management abilities
- Ability to work independently in the community and collaboratively within an interdisciplinary team
- Strong documentation and case management record-keeping skills
