Job Summary
Engage members facing mental health and substance utilization disorders to build relationships supporting open dialogue on sensitive topics. Conduct initial holistic health assessments to identify social determinants of health and cultural influences impacting lifestyle changes. Collaborate with members, caregivers, and providers to develop member-centric case management plans that set goals and address barriers. Monitor and update the effectiveness of these plans to ensure progress toward goals remains pertinent. Facilitate communication among the healthcare team to maximize appropriate care delivery. Provide interventions focused on removing barriers, promoting positive behavior change using motivational interviewing techniques, and stabilizing natural support systems. Maintain concise, accurate, and timely documentation meeting accreditation, contractual, and legal requirements. This telecommuting role supports Premera Blue Cross's whole-person care model for over 2 million customers, offering paid licensure renewal and extensive training in motivational interviewing and LEAN methodology.
Required Qualifications
- Active WA State or Compact license that allows you to conduct an assessment independently
- Behavioral health accepted credentials including LMFT, LICSW, LMHC, LPC Or other CCMC-eligible healthcare licensure
- 3 years' post licensure direct care experience
- 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time
- Certified Case Manager (CCM) certification
- 4 or more years direct care experience across multiple patient populations, conditions, or care settings
- Motivational interviewing experience
- Chronic Condition Certifications (Asthma, Cardiology, Diabetes, High Risk Maternity, Oncology, Pediatrics)
- Integrated Case Management certificate (ICM)
- Chemical Dependency Professional (CDP) certification
- LEAN training
- Individuals residing outside Washington must have or obtain the appropriate Washington State license (apply within 10 days of acceptance)
- Internet access through any Internet Service Provider (ISP) with a minimum connection speed of 75 Mbps (megabits per second) download speed
