Job Summary
Assess, plan, implement, monitor, and evaluate client care actions to meet health and human services needs. Coordinate safe, timely, and client-centered care with offsite support while maintaining caseload productivity through regular client contact. Handle case assignments, draft service plans, and determine case closure with growing independence. Deliver ongoing services including crisis intervention, advocacy, medication monitoring, transportation assistance, budgeting, housing support, and resource referrals. Develop individualized client service plans ensuring compliance with agency policies and document all clinical activities accurately. Participate in treatment teams to recommend client status changes and may lead educational groups or facilitate curriculum. Requires a valid driver's license, auto insurance, and ability to transport clients in a vehicle.
Required Qualifications
- Bachelor's Degree in Behavioral Science or related field
- Knowledge of community resources and systems of care
- Valid driver license
- Auto insurance
