Case Manager - Behavioral Health

Job Summary

Assess client needs, develop individualized service plans, and coordinate behavioral health, medical, housing, and community resources. Support individuals in accessing benefits and services while helping them maintain housing and strengthen independent living skills. Respond to client crises, complete timely assessments and progress notes, and participate in supervision and interdisciplinary team meetings. Partner with healthcare providers, families, and community organizations to promote recovery using person-centered, strength-based, and trauma-informed practices. Travel throughout Hawaiʻi Island for client visits and appointments while maintaining a flexible schedule.

Required Qualifications

  • Bachelor's degree in psychology, sociology, social work, human services, or a related behavioral health field
  • One and a half (1.5) years or more of case management, human services, behavioral health, or mental health experience
  • experience working with individuals experiencing serious mental illness and/or co-occurring challenges
  • Strong communication skills
  • Strong organization skills

  • Strong advocacy skills
  • Strong relationship-building skills
  • Current TB clearance
  • CPR certification
  • First Aid certification
  • willingness to obtain TB clearance, CPR, or First Aid certification
  • Ability to pass a criminal background check
  • Ability to pass drug/alcohol screening
  • Valid Hawaiʻi driver's license
  • current vehicle insurance
  • safety inspection on vehicle
  • Ability to travel throughout Hawaiʻi Island
  • Ability to lift up to 25 pounds
  • Ability to work a flexible schedule

Desired Qualifications

  • Preferred: Bachelor's degree in psychology, sociology, social work, human services, or a related behavioral health field
