Job Summary
Assess client needs, develop individualized service plans, and coordinate behavioral health, medical, housing, and community resources. Support individuals in accessing benefits and maintaining housing while promoting recovery through person-centered, strength-based, and trauma-informed practices. Partner with healthcare providers, families, and organizations to ensure quality, recovery-focused services. Respond appropriately to client crises and complete timely assessments, progress notes, and documentation in accordance with agency policies. Participate in supervision, case reviews, and interdisciplinary team meetings. Travel throughout Hawaiʻi Island for client visits and appointments. Maintain a valid driver's license and ability to lift up to 25 pounds. Work a flexible schedule based on program and client needs.
Required Qualifications
- Bachelor's degree in psychology, sociology, social work, human services, or a related behavioral health field
- One and a half (1.5) years or more of case management, human services, behavioral health, or mental health experience
- experience working with individuals experiencing serious mental illness and/or co-occurring challenges

- Current TB clearance
- CPR certification
- First Aid certification
- willingness to obtain TB clearance, CPR, or First Aid certification
- Ability to pass a criminal background check
- Ability to pass drug/alcohol screening
- Valid Hawaiʻi driver's license
- current vehicle insurance
- safety inspection on vehicle
- Ability to travel throughout Hawaiʻi Island for client visits, appointments, meetings, and community-based services
- Ability to lift up to 25 pounds
- Ability to respond appropriately to emergencies as needed
- Ability to work a flexible schedule based on program and client needs
