Job Summary
Provide individualized substance use disorder case management that empowers patients throughout their recovery journey. Welcome and orient patients to treatment program services, policies, and expectations. Connect patients with internal and community resources through timely referrals and coordinated follow-up to ensure continuity of care. Provide crisis intervention and support during critical moments. Assess patients' social, educational, vocational, and recovery needs to support individualized care. Develop, update, and maintain patient treatment plans and comprehensive records. Complete accurate and timely documentation to support quality care, collaboration, and regulatory requirements. Assist with program implementation, process improvements, and caseload management. Represent the organization through community outreach and engagement activities. Foster a culture of inclusion, respect, collaboration, and belonging. Work 5am - 1:30pm (M-F) with a Bachelor's degree.
Required Qualifications
- Bachelor's degree
- Ability to meet applicable federal and state certification and licensure requirements
Desired Qualifications
- Pursuing certification as a Certified Alcohol and Drug Counselor (CADC)
- Knowledge of addiction, substance use disorder treatment methods, and current trends in opioid maintenance treatment
