Job Summary
Case Manager - Behavioral Health provides condition and case management services to members. Responsibilities include receiving referrals or outreach, conducting comprehensive clinical assessments, leveraging motivational interviewing and behavioral modification techniques, developing and communicating a care plan with member and family, identifying resources and facilitating referrals, monitoring and evaluating the plan over time, ensuring documentation per protocol, and coordinating with the care team and providers to support optimal treatment plans and transitions of care. Requires RN with MN license or Behavioral Health Clinician licensed for independent practice (LMFT, LICSW, LPC, LPCC), 3 years of relevant clinical care experience, CCM Certification or ability to obtain within 3 years. Role is teleworker with full-time remote work, centered in Eagan, MN.
Required Qualifications
- Registered nurse with current MN license and no restrictions
- Behavioral Health Professional licensed for independent practice with one or more licenses: LMFT, LICSW, LPC, or LPCC
- 3 years relevant direct clinical care experience
- CCM Certification or ability to obtain within 3 years of starting in the position
- High school diploma (or equivalency) and legal authorization to work in the U.S.
