Job Summary
Coordinate care as part of a multidisciplinary team to develop and monitor member-centric care plans, utilizing clinical assessments, community resources, and internal/external services to achieve optimal clinical and resource outcomes. Educate members, families, and providers; document all plan activities; outreach to high-risk or high-utilization members; evaluate medical necessity and plan adjustments; may require CCM or CDCES certification within 2 years. The role supports Hybrid Flex or Remote Flex work arrangements with location flexibility based on the Durham headquarters context.
Required Qualifications
- RN with 3 years of clinical and/or case management experience required
- valid and applicable clinical license (NC or compact multi-state licensure) to perform described job duties
- for some roles, additional specialty certification (i.e. CCM, CDCES) may be required
