Job Summary
Case Manager at Avalon Wellness & Recovery Center focusing on connecting individuals with services, resources, and support within substance use disorder treatment. Responsibilities include conducting assessments, developing individualized case management plans, coordinating care with counselors and providers, facilitating access to housing, transportation, employment, healthcare, and benefits, providing ongoing advocacy and support, monitoring progress, maintaining accurate EHR documentation, participating in interdisciplinary care planning, leading educational groups, assisting with discharge planning, and staying informed on community resources and best practices in SUD treatment. Requires experience in behavioral health and SUD, HIPAA/42 CFR Part 2 compliance, and driving as needed.
Required Qualifications
- Minimum of four (4) years of experience in behavioral health, substance use disorder treatment, recovery support services, or case management
- Knowledge of substance use disorders, co-occurring disorders, trauma-informed care, harm reduction principles, and recovery-oriented systems of care
- Strong communication, organizational, and documentation skills
- Ability to establish professional relationships with clients from diverse backgrounds
- Experience utilizing electronic health record systems
- Valid driver's license and reliable transportation
