Job Summary
As the ACT Case Manager, you will provide comprehensive psychosocial services, coordinate care, serve as payee for clients, assist with obtaining services and scheduling appointments, facilitate access to community resources, transportation, and employment opportunities, guide clients in daily living skills, and educate families and partners to maximize benefit usage within an interdisciplinary, onsite team. Responsibilities include managing SSI/SSA payee funds, supporting client goals, linking to housing, medical, income, and other benefits, and leveraging natural supports to encourage treatment engagement. Requires a Bachelor's degree in psychology, social work, or related field (or a BA in another field with behavioral health experience), valid Colorado driver license, reliable vehicle, and BLS certification (or within 30 days). Bilingual skills preferred. Schedule is Monday–Friday, 8:30 am–5:00 pm; fully onsite; salary $21.03–$31.56 per hour, with bilingual differential pay possibility.
Required Qualifications
- Bachelor's degree in psychology, social work or other related human services degree
- Or Bachelor's Degree in unrelated field with at least one year of behavioral health experience
- Valid Colorado Drivers Licenses with a clean driving record
- Reliable personal vehicle
- Must possess a Basic Life Support (BLS) certification or obtain one within 30 days of hire
- Demonstrated interest in the behavioral health field
- Ability to assess crisis situations and intervene appropriately
- Knowledge of community resources
- Basic understanding and ability to work with differing behavioral health presentations
