Job Summary
Provide comprehensive case management services for clients with moderate to high clinical and psychosocial needs, coordinating discharge planning and transitions of care to support continuity. Assess client barriers to recovery, connect individuals with housing, employment, transportation, and community resources, and advocate for their long-term success. Facilitate interdisciplinary case reviews, maintain accurate EHR documentation, and build collaborative relationships with community providers, courts, and agencies. Participate in multidisciplinary team meetings and quality improvement initiatives while ensuring client confidentiality. Requires one year of behavioral health experience, current CPR/First Aid certification, and ability to work in a fast-paced environment. Located in Columbus, IN, this full-time day shift role supports a mission-driven team focused on evidence-based treatment for substance use disorders.
Required Qualifications
- High school diploma or equivalent
- Minimum of one (1) year of experience providing case management, care coordination, or direct services in behavioral health, substance use disorder treatment, mental health, healthcare, or a related human services setting
- Knowledge of community resources and social service systems
- Experience documenting services in an Electronic Health Record (EHR)
- Strong communication, organization, documentation, and follow-through skills
- Ability to work effectively as part of an interdisciplinary treatment team
- Ability to maintain appropriate confidentiality and professional boundaries
- Current CPR/First Aid certification or ability to obtain within 30 days of hire
Desired Qualifications
- Associate's or bachelor's degree in Social Work, Psychology, Counseling, Human Services, Behavioral Health, or a related field
- Two (2)+ years of experience in residential or outpatient substance use disorder or behavioral health treatment
- Experience with discharge planning and transitions of care
- Training in Motivational Interviewing, Trauma-Informed Care, or Recovery-Oriented Systems of Care
- CADAC, Indiana Certified Recovery Specialist (CRS), Certified Peer Recovery Coach (CPRC), Certified Case Manager (CCM), or another relevant behavioral health certification
- Knowledge of Indiana Division of Mental Health and Addiction (DMHA) standards and Indiana community behavioral health resources
