Job Summary
Plan patient discharges from the hospital to home or rehab and coordinate follow-up care to prevent premature readmissions. Connect families with community health programs, financial aid, and necessary medical equipment for home use. Cut hospital costs by preventing long, unneeded stays and work with insurance companies to expedite care approvals. Connect doctors, nurses, and social workers for better teamwork and keep patients and families updated on treatment plans. This Full-time, Monday-Friday role at Ascension St. John's Medical Center in Tulsa, OK, requires a CCMC credential and BLS certification.
Required Qualifications
- High School diploma equivalency with 2 years of cumulative experience
- Associate's degree/Technical degree
- 4 years of applicable cumulative job specific experience
