Job Summary
Full-Time Case Manager at Copa Health in Glendale, AZ, responsible for developing the Integrated Service Plan (ISP), coordinating with healthcare, behavioral health and wellness providers, maintaining progress documentation, and assisting individuals in achieving recovery goals. Works with integrated care teams to engage, educate, and coordinate services, and participates in comprehensive assessments to ensure services prescribed in the ISP are implemented and progress is reviewed.
Required Qualifications
- High School degree or equivalent
- Minimum four (4) years of experience in behavioral health and SMI OR Bachelor's degree in a related field with one (1) year of related experience
- AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements
- Pre-Employment Criminal Background and Drug Testing Required
