Job Summary
Case Manager provides direct support and service coordination to individuals and/or families using a client-centered, strengths-based approach to assess needs, develop service plans, and support clients in achieving goals. Responsibilities include conducting assessments, developing and monitoring plans, meeting with clients, coordinating services and referrals, documenting interactions, advocating for access to services, helping clients develop life skills, collaborating with internal and external partners, maintaining confidentiality, participating in team activities, and performing other duties as assigned.
Required Qualifications
- Bachelor’s degree in social work, human services, psychology, or a related field, or equivalent experience
- Relevant certifications, licensure, or advanced degrees may be preferred
- Prior experience in case management, social services, or working with diverse or underserved populations preferred
- Knowledge of community resources and service coordination practices
- Experience developing and monitoring service or treatment plans
- Ability to maintain accurate documentation and meet compliance requirements
- Strong organizational and time-management skills with the ability to manage multiple clients or priorities
- Ability to work both independently and collaboratively within a team environment
