Job Summary
Screen program applicants for eligibility, conduct assessments, and create individualized service plans. Provide life skills training and job readiness activities to participants while connecting clients with housing, healthcare, and community resources. Monitor client progress, document updates in electronic medical records, and submit data collection and reporting as assigned. Collaborate with nonprofit agencies, housing providers, and educational institutions to strengthen the community resource network and enhance the continuum of care beyond addiction treatment.
Required Qualifications
- Experience in case management
- Knowledge of wraparound services
- Possess strong interpersonal and communication skills
- Ability to present training curriculum in group format
- Ability to develop community resource network
- Computer skills required
