Job Summary
Develop and implement patient care plans while collecting and correlating clinical data to inform the treatment team. Coordinate the continuum of care for assigned patients, ensuring resource utilization and compliance with external agency requirements. Convey medical criteria between insurance providers and the clinical team, and conduct individual or family educational sessions as needed. Manage discharge activities and issues relevant to the team's relationship within the facility. Requires AOD certification from a California-approved organization and a Bachelor's degree in Social Work or Counseling. Full-time hours Monday through Friday from 5:30 AM to 2:00 PM.
Required Qualifications
- AOD Certification is required
- Registration/Certification must be obtained from a California-approved Counselor Certification Organization (CAADE, CADTP or CCAPP)
- Bachelor's degree in Social Work, Counseling or related Human Service field
- 1-2 years of experience working in behavioral health setting
- AOD Registration is required
Desired Qualifications
- Experience with the population served by the clinic preferred
- Previous experience in case management and/or utilization review preferred
- AOD Certification is preferred
