Case Manager and Foster Home Licensor

Conduct family interviews, complete comprehensive home studies within 90 days, and manage youth case plans in collaboration with the child and family team. Maintain phone calls and emails with collateral contacts, process licensing documentation, and coordinate care with Family Support Specialists. Provide necessary support for birth parents, foster parents, and caregivers consistent with the wraparound-service-model while effectively collaborating with the Division of Licensed Resources. Adhere strictly to Agency Policy and Procedure and the Department Operations Manual to meet departmental goals. Commit to culturally competent, trauma-informed, family-focused, child-centered, evidence-based, and data-driven services.
