Job Summary
Case Manager for Housing Works ATI program provides comprehensive case management to court-involved clients, including referrals to healthcare and behavioral health services, treatment planning and reviews, benefits enrollment, documentation in eICare, and coordination with an interdisciplinary team. Responsibilities include weekly client outreach (calls, texts, reminders, letters, home visits), assisting clients with obtaining IDs and vital documents, care coordination, ensuring adherence to court mandates, facilitating weekly compliance reports, maintaining program data and progress notes, building resource lists for external services, facilitating evidence-based groups, and participating in supervision and staff meetings. Requires 1-3 years in human services with criminal justice or behavioral health needs; Bachelor’s Degree; skilled in motivational interviewing, cognitive behavioral techniques, and risk-need-responsivity interventions. Spanish bilingual preferred; strong communication, IT, and trauma-informed approach to care are valued. Familiarity with court systems, OASAS/reentry teams, and data systems (eICare) is beneficial.
Required Qualifications
- Bachelor’s Degree Required
- 1-3 years of relevant experience in human services with individuals involved in the criminal legal system with mental health, substance use or behavioral health needs
- Motivational interviewing
- Cognitive behavioral techniques
- Risk-need-responsivity interventions
- Preferably bi-lingual in Spanish
- Excellent written and oral communication skills
- Proficiency with Microsoft Office and case management systems
- Ability to work independently and with a team
- Trauma-informed approach to care
- Experience in criminal justice field from pre-trial to post-release services
- Ability to lift 25-35lbs
