Job Summary
Monitor consumer well-being across mental, physical, living, learning, working, and social environments by traveling to homes, boarding homes, or schools. Conduct Functional Needs Assessments on each referral, create Individualized Service Plans within 30 days, and provide linkage to community resources. Collaborate with the treatment team, Benefits Specialist, and payee representatives to coordinate internal services and evaluate discharge planning. Maintain accurate clinical documentation, track resource utilization, and ensure compliance with Chapter 106 Medicaid Standards. Manage caseloads effectively while making frequent patient contacts to support psychological adjustment and empowerment.
Required Qualifications
- Bachelor's degree in a mental health discipline
- Must be at least age 23
- Maintain a valid verifiable driver license
- Maintain a clear driving record
- Knowledge of psychiatric disorders in the adult population
- Knowledge of developmental disabilities and substance use in the adult population
- Knowledge of cultural diversity
- Knowledge of behavioral management and crisis intervention
- Knowledge of available community resources and internal services
- Certified by the Alabama State Department of Mental Health to provide adult case management services
