Job Summary
Case Manager for the Long Island Fatherhood Initiative located in Hauppauge, NY. Responsible for outreach, intake, and comprehensive case management to support fathers with economic stability, housing, mental health, and access to related services. Duties include conducting assessments, developing individualized case plans, providing direct services or referrals (in-person, phone, or virtual), coordinating with partners (probation/parole, CPS, shelters, recovery facilities), tracking client progress in nFORM, and delivering engagement activities to promote program participation. Requires coordinating with a team, maintaining detailed records, assisting with reports, and participating in professional development. Must have a valid driver’s license and be willing to travel across Nassau and Suffolk Counties; bilingual Spanish/English strongly preferred. This is a full-time position (35 hours/week, M-F, days) with occasional evening/weekend hours.
Required Qualifications
- Bi-lingual (Spanish/English) strongly preferred
- Bachelor's degree preferred with a minimum of 3 years-experience in healthcare/social service field
- Fingerprint clearance
- Valid driver's license and reliable transportation (MUST)
- Willingness to travel to Nassau & Suffolk Counties
- Good oral and written communication skills
- Good computer skills
