Case Manager (ALF)

Evaluate resident needs and ability of the program to meet those needs at least once every twelve months. Orient new residents to daily routines and assist with resolving adjustment problems. Identify needs and arrange for services and entitlements from public and private sources, including income, health, and social services. Assist residents in obtaining primary medical care and coordinate work of other case management providers. Execute sound discharge or transfer plans and schedule appointments and transportation as necessary. Establish record-keeping systems documenting case management needs and activities. Complete appropriate assessments prior to admission and participate in developing plans of care. Give tours to potential residents and families, and meet regularly with the Sales Manager to ensure inquiry needs are met. Follow up on complaints and grievances in cooperation with the Administrator. Submit monthly reports to the Administrator and attend required meetings including Morning Report and Admission Committee.
