Job Summary
Case Manager responsible for timely and efficient allocation of support workers to participants receiving Support at Home, coordinating client visits and worker assignments, reviewing care plans, advising on complex care situations, and managing budgeting and resource utilisation to meet individual goals within the Aged Care/NDIS context in Fairfield, NSW. Collaboration with participants, families, and healthcare professionals is required, with emphasis on scheduling, geolocation factors, and evidence-based practice to deliver high-quality care across regulated environments.
Required Qualifications
- Experience in Support at Home or relevant sectors
- Knowledge of the NDIS and/or Aged Care sector
- Excellent communication and time management skills
- Confident computer skills (rostering systems or CRMs a bonus)
- The right to work unrestricted in Australia
- Police Check, WWCC, NDIS Worker Screening Check (or willingness to obtain)
- Current First Aid & CPR (or willingness to obtain)
- Minimum of two doses of COVID-19 vaccination
- Comfortable in fast-paced, regulated, or changing environments
- Proactive and committed to continuous improvement
