Job Summary
Case Manager for Adult Protective Services in NYC overseeing crisis intervention, protective services, and social casework for older adults. Manage complex caseload from emergency triage to long-term protective planning; conduct crisis triage and intake, in-home and office assessments, develop written case plans (including counseling, benefit screening, and arranging services such as Home Health Aides, Legal Aid, and Housing), assess capacity and assist with financial management or guardianship to prevent elder abuse, conduct ongoing field monitoring with on-call rotations, and complete required documentation using specialized software. Strong emphasis on professional judgment, elder justice, collaboration with leadership and supervisors, and potential supervision of aides, interns, and volunteers. Requires education (BSW or BA/BS), computer literacy, clear English communication; multilingual ability is helpful; requires ability to travel to client homes; alignment with a nonprofit mission and flexible hybrid work setup.
Required Qualifications
- Graduate of an accredited college or university with a BSW or BA/BS (Psychology or Gerontology preferred)
- Technical skills: Computer literacy and meticulous record-keeping
- Proficiency in English; knowledge of Yiddish, Russian, Spanish, or Patois is helpful
- Ability to travel to clients’ homes using mass transit and navigate stairs or other barriers
- Commitment to elder justice and understanding of diverse client contexts
