Job Summary
Conduct preliminary screenings, triage, and emergency interventions for older adults requiring protective services. Perform in-home and office assessments using standardized instruments to evaluate social and emotional needs. Develop written case plans for counseling, benefit screening, and service arrangements including home health aides and legal aid. Evaluate client capacity to manage affairs and assist in securing financial management or guardianship to prevent exploitation. Complete ongoing in-home visits to monitor risk, evaluate functioning, and revise case plans in real-time. Participate in a rotating on-call schedule for evenings, weekends, holidays, and crisis situations requiring emergency medical or court-ordered actions. Submit required case records, reports, and statistics within mandated timeframes using specialized software.
Required Qualifications
- BSW or BA/BS
- Psychology or Gerontology
- Computer literacy
- Ability to maintain meticulous records
- Proficiency in English
- Ability to travel to clients' homes using mass transit
- Ability to navigate physical barriers (stairs, etc.) in residences
- Ability to understand and appreciate the roles of race, religion, ethnicity, and sexual orientation in serving a diverse client base
