Job Summary

Develop service plans, link and coordinate community services, and advocate for consumers to assess needs and obtain concrete benefits like SSI and Medicaid. Conduct re-assessments, monitor service delivery monthly, and ensure continuity of care across multiple providers while supporting coping skills to prevent hospitalization. Participate in person-centered treatment planning, maintain case records, and prepare reports to meet agency productivity and documentation standards. Work with professional independence under the supervision of the Adult Outreach Services Manager to enhance consumer participation in aftercare and support services.

Required Qualifications

  • Bachelor's Degree in Social Work, Psychology, or related field
  • Ability to be credentialed as a QMHP (Qualified Mental Health Professional)

Desired Qualifications

  • Current CPR/First Aid Certification, preferred
  • 1 year experience working with adults and families, preferred
