Job Summary
Case Manager role at ADAPT Community Network focusing on developing, implementing, and monitoring Life Plans for residents in NYC-area settings. Required Qualifications
- Bachelor’s Degree in Human Services or related field required
- Minimum of one (1) year of experience working with individuals with developmental disabilities required
- Ability to read, write, speak, and understand English
- Strong interpersonal and communication skills
- Excellent organizational and time management skills
- Proficiency in Microsoft Office and electronic record systems
- Ability to maintain confidentiality and exercise discretion
- Ability to work independently, as part of a team, and with flexibility in scheduling
- Ability to work flexible hours
