Case Manager, Acute Rehabilitation - Per Diem, Primarily Mon-Fri

Conduct admission reviews for acute rehabilitation patients, identifying psycho-social barriers and discharge planning needs to meet continuum of care goals. Complete ongoing assessments of discharge requirements, mitigate barriers, and facilitate safe, timely transitions by collaborating with interdisciplinary teams and arranging post-discharge services. Remain in contact with patients and families at least three times weekly to ensure they remain informed of the rehab process and discharge plans. Facilitate authorizations, pre-certifications, concurrent reviews, and retro authorizations while maintaining clinical records according to regulatory guidelines. Participate in daily stand-up and weekly interdisciplinary team meetings to review patient progress and provide updates on care plans.
