Job Summary
Design and implement care plans for patients with behavioral health conditions as part of a treatment team. Assist patients in accessing healthcare services and resources to meet identified goals, focusing on improving overall quality of life through health, relationships, and meaningful activities. Provide exceptional customer service to internal and external parties while maintaining high attention to detail. Utilize a trauma-informed, patient-centered approach to support patient recovery. Requires current driver's license, advanced Microsoft skills, and ability to travel between locations. Flu immunization, TB screening, de-escalation training, and BLS certification required within 90 days.
Required Qualifications
- Bachelor's degree in health or human service-related field
- Minimum of 1 year in health or human services setting
- Current driver's license
- Ability to be commercially insured
- Driving record that allows insurability with AHS commercial insurance agency
- Advanced Microsoft computer skills
- Ability to provide exceptional customer service for internal and external parties
- Excellent interpersonal skills
- Demonstrated ability to work effectively both independently and as part of a team
- High attention to detail
- Annual Flu immunization
- Annual TB screening
- De-escalation training within 90 days of hire
- BLS certification required within 90 days of hire
- Ability to occasionally lift and/or move 15 pounds up to 50 pounds
- Ability to travel between AHS locations
