Job Summary
Provide comprehensive, participant-centered case management services to a caseload of 120 to 150 individuals, including training participants, job search candidates, and alumni. Maintain regular contact through individual meetings, phone calls, and virtual appointments to monitor progress, while utilizing trauma-informed and strength-based techniques to promote engagement and goal attainment. Connect participants to community resources for housing, benefits, healthcare, and childcare, and facilitate workshops on wellness and life skills. Collaborate with instructors and staff to support retention and employment outcomes, maintaining accurate case notes in Salesforce. Manage a diverse caseload with weekly travel between program sites and partner locations, working a minimum of four days per week across all HOPE and partner locations.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or comparable educational background or training
- Experience in case management or supportive services
- Knowledge of trauma-informed care and New York City resources and services available to support participants
- Strong communication, organization, and time management skills
- Experience facilitating workshops or group sessions
- Proficiency in Google Workspace
- Ability to travel between program sites
- Professionalism, cultural humility, and a commitment to supporting diverse populations
- Work a minimum of four days from the office across all HOPE and partner locations
- Work schedule will include evening and weekend hours
- Travel weekly between program sites and community partner locations, as required
