Job Summary
Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills. Provide direct care for ADLs and group activities; lift and transfer individuals as appropriate to their needs. Coordinate services with Interdisciplinary Team. Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams. Develop and implement Day Habilitation Service Plans (DHSP), including maintaining progress notes and documenting progress on objectives; write bi-annual and annual summaries; complete required documentation in a timely and accurate manner. Participate in supervision sessions, program-wide staff meetings and case review meetings; participate in team meetings regarding general program development, including curriculum development and schedules. Demonstrate good computer skills and ability to learn agency programs for documentation. Attend in-service trainings; ability to work in a team environment; multitask and prioritize tasks; strong written and verbal communication skills; comply with state and federal regulations and agency policies; protect health and safety. Perform other duties as requested. Requirements include experience with ID/DD and a degree path as described above. Employment at Lifeworks is at-will; equal opportunity employer.
Required Qualifications
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License required
- Valid MAP certification is preferred
