Job Summary
Case Manager responsible for assisting individuals served with ID/DD to obtain employment aligned with goals; collaborate with team to plan advocacy and support; support self-employment, education, social growth, health and wellness, and community participation; manage electronic and physical case records; develop and implement Individual Support Plans (ISP) and maintain documentation; generate progress notes; provide on/off-site Life Enhancement Curriculum support; lead community volunteer and learning experiences; daily payroll/billing tasks; additional duties as needed at various locations. Requires strong communication, independence, and commitment to empowering adults with developmental disabilities.
Required Qualifications
- Experience working with Individuals with ID/DD
- Bachelor's degree or an associate degree in a related field with two years of experience OR HS diploma with five years of related work experience in Human Services
- Valid Driver's License
- Ability to pass Fingerprint background check, CORI, DPPC and Driving background checks
- MAP certification preferred
