Job Summary
Case Manager for individuals with ID/DD in Lifeworks’ residential programs. Responsibilities include promoting health and safety, supervising and coaching across Life Enhancement activities, developing and presenting annual Individual Support Plans (ISPs), performing documentation and file maintenance, coordinating transportation, and ensuring effective communication with the ISP team and individuals served. Requires a Bachelor's or Associate degree (or HS diploma with substantial related experience), strong communication skills, and a valid driver’s license; MAP certification preferred. This is a full-time role in West Roxbury, MA with benefits and growth opportunities.
Required Qualifications
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
- Valid Driver's License required
- MAP certification is preferred
- Excellent verbal and written communication skills
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Basic computer skills; ability to learn new systems
