Job Summary
Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills; provide direct care for ADLs and group activities; lift and transfer individuals as appropriate to their needs; coordinate services with Interdisciplinary Team; communicate respectfully with families, residential staff and participants’ teams; develop and implement Day Habilitation Service Plans (DHSP); maintain progress notes and participant records; complete required documentation in a timely and accurate manner; participate in supervision sessions, staff meetings, and program development; demonstrate strong computer skills for documentation and data; comply with regulations and agency policies; diverse responsibilities as assigned.
Required Qualifications
- Experience working with Individuals with ID/DD
- Bachelor's degree or associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services
- Valid Driver's License
- MAP certification is preferred
- Ability to pass background checks (Fingerprint, CORI, DPPC) and driving background checks
- Excellent verbal and written communication skills
