Job Summary
Case Manager for adults with developmental and intellectual disabilities in a nonprofit setting. Responsible for helping individuals served obtain and maintain employment aligned with their goals; collaborating with a team to plan advocacy and support; supporting self-employment, education, social growth, health/wellness, and community activities; maintaining electronic and physical case records; writing and implementing Individual Support Plans (ISP) and completing required documentation; generating weekly/monthly progress notes; providing on/off-site support for Life Enhancement Curriculum; leading community volunteer and learning experiences; completing payroll and billing documentation; and performing additional duties as needed at diverse locations. Requires experience with ID/DD populations, strong communication skills, and driving credentials.
Required Qualifications
- Bachelor's degree or associate degree in related field with two years of experience, OR HS diploma and five years of related work experience in Human Services
- Valid Driver's License
- Fingerprints background check, CORI, DPPC and Driving background checks
- MAP certification is preferred
- Excellent verbal and written communication skills
