Job Summary
Discharge-planning focused Case Manager leading the clinical team to identify patient needs for discharge and connect patients experiencing homelessness with housing options and community resources. Responsibilities include developing comprehensive discharge plans, coordinating with community agencies for services (vocational rehabilitation, mental health, substance abuse support, job readiness, and parenting skills), accompanying patients on site visits to clarify discharge plans, assisting with entitlements and applications, and maintaining thorough documentation. Requires strong interpersonal and communication skills, bilingual Spanish desirable, valid Arizona driver’s license, and ability to work with culturally diverse, medically needy populations. This is a safety sensitive role with no supervisory responsibilities.
Required Qualifications
- Associates Degree or equivalent experience required; BSW preferred
- 3 – 5 years of experience preferred
- Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors
- Experience in working with culturally diverse, chronically homeless, and medically needy individuals preferred.
- Demonstrates excellent oral and written communication skills
- Bilingual skills – Spanish highly desirable
- Must demonstrate critical thinking, problem-solving, and organizational and time management skills
- Possess and maintain a valid Arizona driver’s license and has use of privately owned vehicle
