Job Summary
Manage a caseload of up to 25 participants in the Bridges to Success Adult Reentry program, utilizing the Housing First approach to assist formerly incarcerated individuals in securing housing stability and community reintegration. Navigate clients through the Coordinated Entry System, conduct intake assessments, and coordinate services with health care providers, employment offices, and treatment programs. Maintain client files, develop service plans, and provide daily, weekly, and monthly reports within WestCare's Clinical Data System. Assist with housing paperwork, conduct community outreach, and monitor progress to ensure client sustainability. Requires a two-year degree in social work or related field, two years of case management experience, and valid driver's license.
Required Qualifications
- Minimum two (2) years' experience in social services, case management, or related human services field.
- Strong knowledge of housing programs, supportive services, and community resources.
- Excellent communication, public speaking, and interpersonal skills.
- Strong computer skills, including Microsoft Office (Word, Excel, PowerPoint).
- Good documentation and organizational skills.
- Ability to work independently, manage multiple priorities, and remain calm in challenging situations.
- Demonstrated ability to work effectively with diverse and underserved populations.
- Valid driver's license and ability to transport clients.
- CPR Certification, First Aid Certification, and annual Tuberculosis test.
- Graduation with a two-year degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling or a related field
- High School diploma or equivalent
