Job Summary
Case Manager for The Doe Fund’s RWA program based at 510 Gates Avenue, Brooklyn. Manage a caseload of clients, monitor progress toward service-plan goals, and coordinate access to external referrals for Mental Health, Substance Recovery, Benefits, Entitlements, and Primary Care. Communicate with DHS and other agencies as needed; collaborate with clients and staff to support goal setting, employment, housing, graduation, retention, and related program goals. Organize and facilitate caseload and group activities; maintain accurate documentation in client-tracking systems and DHS CARES. Provide crisis intervention as needed and perform other duties. Requires a High School Diploma (Bachelor’s degree preferred) and at least 2+ years in Human Services/Case Management; CASAC a plus; strong communication skills; MS Office/Google proficiency; bilingual preferred.
Required Qualifications
- A High School Diploma is required.
- Bachelor’s degree in Psychology, Sociology, Counseling, Social Work, or other Human Services concentration preferred.
- At least 2+ years of experience in Human Services/Case Management
- Experience working with challenging high-service needs populations preferred
- CASAC a plus
- Strong interpersonal and written/verbal communication skills
- Working knowledge of MS Word, Excel, PowerPoint, and Google
- Bilingual preferred.
