Job Summary
The Case Manager will manage a caseload of people living with AIDS, providing outreach and case management while addressing issues such as homelessness. Responsibilities include linking consumers to supportive services, maintaining documentation of outreach efforts, and coordinating holistic care services. Candidates need a bachelor's degree in Psychology, Social Work, or a related field, along with two years of case management experience. Bilingual skills in Spanish are preferred, and CPR and First Aid certifications are required.
Required Qualifications
- Two years case management experience
- Valid driver's license
- Ability to obtain and maintain Fresno County Credentialing
- Full vaccination against COVID-19 as mandated
- Successful completion of a pre-employment drug test
- Pre-employment physical and criminal background check
Desired Qualifications
- Bilingual Spanish/English preferred
- Prior experience working with the homeless
- Experience in outreach, substance abuse, and/or mental illness
- Excellent communication skills
- Basic computer skills
- Good documentation skills
- Ability to multi-task
- CPR Certification
- First Aid Certification
- Annual Tuberculosis test
