Job Summary
Promote optimal health and ensure safety for adults with developmental disabilities through routine, preventive, and emergency care. Supervise, train, and coach individuals across Life Enhancement Program activities to increase self-sufficiency while providing education, counseling, and physical assistance for employment, social growth, and community mobility. Develop annual Individual Support Plans, document training and activities, and maintain confidential files for assigned caseloads. Act as a liaison between Lifeworks, employers, volunteer sites, and families to facilitate consistent programming and support. Provide transportation to community sites using agency or personal vehicles. Maintain regular communication with ISP team members and persons significant to the individual served.
Required Qualifications
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License
- Excellent verbal and written communication skills
