Job Summary
Case Manager to support adults with developmental disabilities in Norwood, MA. Responsibilities include developing and presenting annual Individual Support Plans (ISP), providing supervision, training, and coaching across Life Enhancement Program activities, educating and counseling individuals to achieve goals in employment, education, social growth, health and wellness, and community participation, maintaining confidential case files, coordinating with families, employers, and community sites, and arranging transportation as needed. Requires a Bachelor’s or Associate degree (or HS diploma with extensive related experience), valid driver's license, and ability to pass required background checks. Strong organizational, documentation, and communication skills are essential; position supports a nonprofit human service mission with competitive pay and benefits.
Required Qualifications
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
- Valid Driver's License required
- Fingerprint background check, CORI, DPPC and Driving background checks
- MAP certification is preferred
- Excellent verbal and written communication skills
- Experience working with Individuals with ID/DD
