Job Summary
Case Manager for a homeless shelter in Brentwood, NY. Responsible for completing new client intakes, explaining shelter rules, meeting clients twice weekly, developing Independent Living Plans, coordinating housing resources with Housing Specialists, conducting safety inspections, supporting daily living skills, maintaining HMIS documentation, and preparing monthly reports. Role requires advocacy, counseling, and connection to community-based resources, with transportation as needed and collaboration across program teams.
Required Qualifications
- High school diploma required.
- Bachelor’s degree in Human Services or related field preferred
- Minimum of 2 year of experience in human services or related field preferred
- Knowledge of the Suffolk County Department of Social Services preferred
- Knowledge of human services and previous experience working with at-risk population preferred
- Excellent interpersonal, verbal, and written communication skills required
- Strong organizational and time management skills required
- Proficient computer skills, including Microsoft Office required; experience with HMIS, Google Apps, and web-based project management software preferred
- Bilingual in Spanish preferred
- Valid and clean NYS Driver's License required
