Job Summary
Case Manager position supporting adults with mental illness and co-occurring substance use disorder in rehabilitation and recovery services; responsibilities include developing and maintaining treatment plans, providing 1:1 or group counseling, documenting progress, coordinating services to meet discharge planning goals, facilitating skills training (including self-advocacy, ADL, medication management, socialization, and conflict resolution), encouraging adherence to appointments, supervising medication management and medical appointments, accompanying clients to medical/social services locations, liaising with criminal justice and community providers, arranging activities and resources, conducting crisis assessments/interventions, and ensuring compliance with policies and documentation requirements; requires Bachelor's degree in human services or equivalent combination with relevant experience, and proficiency in software like IMA, Word, and Outlook.
Required Qualifications
- Bachelor's degree in a human services field*
- Associate’s Degree in a human services field* and two years of full-time experience providing direct services to individuals with mental disabilities or other relevant human service experience
- High school diploma or equivalency diploma recognized by the NYS Department of Education, and four years of full-time human services work experience
- An equivalent combination of training and experience. 30 college credits may be substituted for 1 year of human services experience.
