Case Manager 2 - Middlesex Pilots (Middletown)

Deliver comprehensive case management services to clients, including intake assessments, development and implementation of individual care plans, and coordination with treatment providers and community resources. Support clients' recovery from mental illness, substance use, and homelessness within the Residential Behavioral Health Treatment care team. Requires an Associate's Degree, two years of related experience in direct service and case management, and a valid Connecticut Driver's License.
