Job Summary
Develop person-centered plans of service and conduct care coordination through telephonic outreach and on-site visits for clients in the Medical Day Care Services Waiver Program. Evaluate client needs using assessments, maintain database integrity, and document coordination activities in multiple computerized systems while ensuring compliance with COMAR regulations and contract requirements. Collaborate with community agencies to connect clients to resources, triage inquiries from families and providers, and report suspected abuse or neglect to the supervisor. Submit reportable events to MDH, educate clients on advocacy and technology use, and manage schedules to meet minimum daily documentation and service hour requirements.
Required Qualifications
- Experience as a Supports Planner or CCS
Desired Qualifications
- Bachelor's degree in human service focus (psychology, social work, public health, etc.)
- Experience working with medical-based community resources
