Job Summary
Coordinate individualized discharge plans for active-duty patients in inpatient behavioral health treatment by collaborating with multidisciplinary clinical teams and addressing barriers to discharge. Develop and implement treatment plans using patient charts and aftercare assessments, while scheduling follow-up appointments, referrals to PHP or IOP, and family or outpatient treatment team meetings. Connect patients with community resources such as crisis homes and mental health practitioners to support post-discharge needs, and communicate discharge plans effectively to patients, families, and external agencies. Document all patient interactions, treatment plans, and discharge planning activities accurately in medical records, perform administrative tasks including report preparation and maintaining legal logs, and attend clinical treatment team meetings to provide updates. Assist with utilization review processes to ensure compliance and continuity of care.
Required Qualifications
- Bachelor's degree in Psychology, Social Work, or a related clinical field
- Strong oral and written communication skills
- Ability to work independently and collaboratively within an interdisciplinary treatment team
- Familiarity with community resources relevant to behavioral health patients
- Knowledge of chronic mental illness and associated care challenges
- Understanding of legal holds (e.g., 5150, 5250, 5270)
Desired Qualifications
- At least one year of work experience in a behavioral health setting (pre- or post-graduation)
- Demonstrated interest in behavioral health and patient care coordination
- Experience with discharge planning and case management
- Proficiency in administrative tasks, such as report preparation and documentation
