Job Summary
Design and implement individual care plans for clients in accordance with policies, rules, regulations, and directives. Utilize treatment approaches and case coordination to work with a diverse client population, providing service coordination as assigned. Maintain HIPAA compliance and follow organizational confidentiality and security rules while communicating effectively with internal and external customers. Operate standard office equipment and Windows-based applications to manage case information. This full-time role in Montrose, CO, supports the Crisis Walk-In Center within the integrated behavioral health program. The position requires a driver's license, current flu immunization, and completion of de-escalation and BLS training within 90 days.
Required Qualifications
- Bachelor's degree in one of the recognized mental health professions
- Previous experience with substance abuse and mental health related case management and treatment
- Must be knowledgeable regarding patient health information and HIPAA compliance
- Must be organized and demonstrate the ability to follow guidelines
- Must demonstrate excellent interpersonal and communication skills
- Must demonstrate the ability to be self-directed
- Must demonstrate the ability to follow the organization's confidentiality and security rules
- Must have current driver's license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis Health System's insurance agency
- Annual Flu immunization
- Annual TB screening
- de-escalation training within 90 days of hire
- BLS certification required within 90 days of hire
- Occasional travel required
Desired Qualifications
- Post graduate degree (Masters) in one of the recognized mental health professions
- Two years human service-related experience
