Job Summary
Coordinate administrative duties for Primary Care Provider teams and Integrated Care Teams, serving as the primary contact between customer-owners and clinical staff. Manage complex referrals, schedule appointments and surgeries, and process incoming calls while scanning documents into provider charts. Facilitate communication between providers, coordinate travel and housing, and assist with treatment scheduling across specialties including dental, pediatric, oncology, and rehabilitation. Support care coordination for children and families by connecting them to developmental services and therapies, and manage high-volume workloads involving 1,200 customer-owners in primary care. Process referrals, authorizations, and resource sharing to ensure timely access to specialized clinical care and wellness services.
Required Qualifications
- High School diploma; OR GED
- Bachelor's degree
- four (4) years of administrative support experience
- equivalent combination of education and experience
- demonstrated proficiency as an Administrative Support II at SCF
- Two (2) years of Case Management Support experience
- demonstrated as a Case Management Support I at SCF
