Job Summary
Case Management Support I/II coordinate and manage administrative duties for a Primary Care Provider (PCP) team and the Integrated Care Team (ICT); responsibilities include scheduling coordination, verbal and written communications, information research and transfer, and system coordination. The role provides progressive responsibilities between levels I and II and requires familiarity with Alaska Native populations and culturally informed care. Health screening and required immunizations for Health Care Personnel are prerequisites, with a native-preference hiring policy noted.
Required Qualifications
- High School diploma; OR GED.
- Bachelor’s degree; OR four (4) years of administrative support experience; OR equivalent combination of education and experience; OR demonstrated proficiency as an Administrative Support III at SCF.
- Two (2) years of Case Management Support experience; OR demonstrated proficiency as a Case Management Support I at SCF.
