Job Summary
As a Case Manager Supervisor, you will supervise community-based case managers to help provide collaborative and comprehensive services impacting adults with persistent mental illness, focusing on reducing hospitalization frequency and length, increasing community resource connections, and improving quality of life. Responsibilities include working collaboratively with the Clinical Team, providing on-call crisis intervention, maintaining accurate consumer clinical records, and overseeing service provision from multiple providers. Essential skills include assessment, crisis intervention, and population-specific communication.
Required Qualifications
- Master’s Degree in field with clinical training
- Bachelor’s degree (BA, BS, BSW) in a mental health related field
- One (1) years of post-bachelor’s degree experience providing direct services to adults with a serious mental illness
- One (1) year of relevant supervisory experience preferred
- Valid driver’s license in good standing
- Pre-employment drug screening
