Job Summary
Supervise cases for At-Risk Youth and individuals with intellectual deficits, providing weekly direction, support, and counsel to the case management team. Manage team operations including performance evaluations, scheduling, orientation, training, and personnel decisions such as hiring, transfers, promotions, and discipline. Determine caseloads, assign cases, review treatment plans for accuracy, track performance data, and ensure file compliance. Interview served individuals for medical history, conduct intake, administer diagnostic tests, and document results. Assist in developing Individual Service Plans, consult with medical specialists on treatment changes, and administer therapeutic treatments like behavior modification and anger management. Refer individuals to supportive services, conduct supervisory home visits, and provide on-call coverage for rotational emergency crisis intervention. Ensure RBWO waivers and SCORE updates are completed and submitted promptly.
Required Qualifications
- Bachelor's Degree in related field", "Master's degree in Social Services if required by state", "Two to four years of experience in human services", "Minimum of one year of experience as a Case Manager", "Current CPR/First Aid Certification as required by state/program", "Self-motivated and detail-oriented with ability to multi-task", "Strong analytical and communication skills with ability to build rapport with a team", "Ability to exercise sound judgment and independently problem-solve
